Developmental Pathways has a great opportunity for a Registered Nurse (RN) who is looking to make a difference. This position will provide administrative support for the development of the home health agency. As our RN, you will get to be involved in the development of nursing documentation and training, assisting in development of Quality Assurance and Performance Improvement program as well as establishing relationships with referral sources. This position will provide limited nursing care in accordance with the patient's plan of treatment, including comprehensive health and psychosocial evaluation, monitoring of the patient’s condition, health promotion and prevention, coordination of service, parent teaching and training activities, provision of direct nursing care and planning for patient’s needs or service requests to individuals in service within their home or community environment. You will get to team up with others to develop and implement goals and strategies to enhance the child’s overall development.

More about what you’ll get to do as our RN

  • Assist in the development of and assure full compliance with agency policies and procedures
  • Assure full compliance with state and federal regulatory guidelines
  • Assist in development of nursing documentation within Electronic Medical Record (EMR) system and managing workflows within the EMR
  • Perform initial and on-going medication reviews for patients admitted to the Home Health Agency (HHA)
  • Assist in the development and carry-through of the agency’s Quality Assurance and Performance Improvement (QAPI) program, including managing the Infection Control program
  • Maintain records and reports while also assisting with chart reviews of documents
  • Communicate with physicians and staff regarding orders including recording verbal orders or picking up signed orders from physician offices, as necessary
  • Initiate the skilled nursing plan of care and revises as necessary in consultation with the physician
  • Initiate appropriate preventive and rehabilitative nursing procedures, as appropriate
  • Serve as coordinator of patient's care including referral to other services as appropriate
  • Teach, supervise, and counsel the patient and family members regarding medication management, the disease process, self-care techniques, prevention strategies, and other related problems of the patient; promote individual/family independence
  • Assist with Emergency Preparedness documentation and staff training
  • Assist with planning for the patient's discharge from service
  • Participate on Governing Body and Professional Advisory Committee for HHA
  • Partner with therapists, service coordinators, doctors and other community partners who serve young children to deliver early intervention

Starting Salary and Benefits

Requirements to be our RN

  • Registered Nurse (RN) license for the State or Colorado
  • Recent, pertinent continuing education in accordance with licensure regulations
  • Knowledge of Colorado Department of Health & Environment rules and regulations 
  • Familiar with Microsoft Office (Excel, Word, Outlook, etc.) and be able to use these and other required (Salesforce, KanTime or other EMR, etc.) programs at an intermediate level
  • Excellent oral and written communication skills
  • Ability to work flexible hours including evenings and weekends to accommodate family schedules and on-call coverage to support HHA
  • Able to work cooperatively and collaboratively with a diverse group of individuals and agencies
  • Personal vehicle in good operating condition for use during work as appropriate
  • Colorado driver's license/proof of auto insurance
  • Meet our driving requirements; no suspensions/revocations and less than three (3) tickets in the past three (3) years


  • Minimum of one (1) year of home-care nursing experience, preferred
  • Working knowledge of Centers for Medicare and Medicaid Services and Accreditation Commission for Health Care rules and regulations for home health care, preferred
